Supported living and community outreach services

Our supported living and community outreach services offer different models of support for an individual. Whether in someone’s own home, flat schemes or shared accommodation, we have a range of individual flexible packages available from 24 hours, to just a few hours of support per week within the community, depending on your needs.

We encourage and support you to live a full and varied life, including;

  • Keeping you safe and healthy
  • Developing your domestic and life skills e.g. shopping, cooking, cleaning etc.
  • Helping you to manage your money
  • Providing you with sound, trusted advice and advocacy
  • Being part of your community e.g. social groups, attending local events and access to other local services
  • Supporting your aspirations to learn new things e.g. to attend college, work placements, volunteering and paid work

Complex care

We offer a wide range of specialist care and support services to adults with complex needs. Our complex care services ensure the people we support maintain independence and choice.

Complex care support is planned around people’s lives, to ensure dignity and respect. Recognising complexity, and working in partnership, we aim to ensure our support is consistent, long term and of the highest quality. We also understand the sensitivities of providing support in your own home.

Individual needs, dignity and respect
