Specialist support for adults with a learning disability

Our learning disability offer

We provide specialised, person-centred support for adults with a learning disability who may also have additional needs – whether that’s in the comfort of their own home or in accommodation we offer in partnership with trusted housing providers.

Over the past 30 years we’re proud to have supported thousands of people to achieve their own personal goals, gain independence and live full and varied lives. From everyday supported living through to more enhanced requirements, we provide a holistic approach that includes personal care, health and wellbeing and emotional support.

Our skilled and experienced teams work with every person we support to create a personalised, outcome-focused plan to achieve their aspirations.  With high retention of our staff, we pride ourselves on offering a consistent and high-quality level of support.

Supported Living

Our supported living service provides 24-hour care that enables people with the most complex needs to live as independently as possible. 

We can provide this support in the comfort of the person’s own home or in the accommodation we provide in partnership with local housing providers.

We work closely with trusted housing providers to offer high quality accommodation, ranging from flat schemes to shared houses depending on the needs and wishes of the person supported. 

Homes are set within the heart of communities, close to local amenities and transport links. We consider the use of assistive technology to further aid independent living.

Depending on the property, some areas may be shared such as the living room, dining room and kitchen. We encourage the people we support to contribute to decisions about their home, from soft furnishings to how it is decorated. 

Community Outreach

Our community outreach services offer support to people who require less than 24-hour support in their own homes. 

We offer flexible support packages with as much or as little support as needed, whether that’s a few hours a day or week for things like shopping or help with tasks around the house, or an intensive period to work towards a specific goal.

How We Help

Our experienced teams offer bespoke support for people to: 

  • Stay safe and healthy by providing support with eating a balanced diet, exercise and medical appointments 
  • Develop life skills such as shopping, cooking and cleaning
  • Budget and manage money by signposting people to specialised advice 
  • Get support to express their views and wishes – we can arrange for an impartial advocate to support with important decision making for those than need it  
  • Be an active participant in their local community – whether that’s through volunteering, attending community events or enjoying local activities
  • Get in to work by providing support with job applications, interviews and any ongoing needs
  • Take up hobbies and learn new skills, whatever the interest or passion
  • Enjoy an active social life with support to spend time with friends, family and loved ones 

The process

Initial assessment

Once a referral has been made through the appropriate channel, we will carry out a comprehensive assessment with the person requiring support, their family, and other professionals involved in their care. We will work to fully understand their needs and goals and identify what the right support looks like.

Support plans

We work collaboratively with an individual to build a personalised support plan, in line with Care Quality Commission (CQC) fundamental standards.

We will learn the person’s history and circumstances, their needs, interests and skills, and include them, their families and carers (where appropriate) to identify clear goals and outcomes.

Ongoing support

We recognise the needs of the people we support are constantly changing and developing. We hold monthly progress meetings and formal annual care reviews to discuss what is going well, what might need to change and to track progress towards achieving personal outcomes.
