We support young people with learning disabilities and autism to transition into adulthood with confidence, independence and meaningful opportunities.
About Our Supported Living Opportunities
We work closely with individuals, families, colleges, and professionals to create personalised support that reflects each person’s goals, interests, and aspirations. We currently have a range of supported living opportunities across:
- Cheshire East
- Lancashire
- Liverpool City Region
Alongside our existing homes, we can also work creatively with families, commissioners and housing partners to develop bespoke flat schemes and tailored supported living opportunities across the North West. This allows us to create environments that truly meet the needs of young people as they move into adulthood and greater independence.
Goals and outcomes
Every person we support is unique, which is why we take a person-centred approach to creating goals and outcomes. We work alongside individuals to understand their wants, needs, interests and future aspirations.
Together, we identify opportunities within the local community that help people build confidence, independence and life skills. This may include:
– Volunteering opportunities
– Work placements and employment pathways
– Social and leisure activities
– Independent travel training
– Developing daily living skills
– Building relationships and community connections
Our aim is to support people to live fulfilling lives, make choices and become as independent as possible.
Co-production
Co-production is at the heart of everything we do. We believe the people we support should have a genuine voice within our organisation and play an active role in shaping services.
Our dedicated Co-Production Lead works closely with the people we support to ensure their views, experiences and ideas influence how we work. This includes involvement in:
– Providing feedback about services
– Inputting into policies and procedures
– Supporting recruitment and staff training
– Quality checking services
– Helping shape future developments
By listening and working together, we ensure our services continue to reflect what matters most to the people we support.
David’s Story
David came to us from Seashell Trust and has been building his confidence and independence step by step since starting support in December 2024. When our team first met David at Seashell, we took time to get to know him properly, understand what was important to him and build a support plan around the things he enjoys and wants to achieve.
David now receives outreach support while living with his dad in Rochdale and is working towards his goal of moving into supported living in the future. He enjoys bowling, mini golf, swimming, gaming, and going out for meals and chooses activities he wants to do each week. Staff are also supporting him to build everyday independence skills like shopping, cooking meals, household tasks and trying new activities.
David has a great sense of humour and has built really positive relationships with his support team. As his confidence has grown, he’s become more comfortable communicating, trying new things, and getting involved in the community. He currently volunteers at a leisure centre reception and also attends a work placement at Seashell, with hopes of moving into paid employment in the future.
David’s dad says it has been amazing to see how relaxed, happy, and confident he has become, especially seeing the friendships and banter he now has with staff. David’s journey shows how the right support, encouragement, and opportunities can help young people grow in confidence and move towards a more independent future.
Harry’s story
Harry is a young man who transitioned into adult supported living at the age of 18. He now lives in Burnley and has made great progress with the support of his team.
